I included assessment findings, nursing diagnoses, interventions, outcomes, and evaluation. I included nursing research and theory to integrate knowledge of my patients&#8217; admitting diagnosis to provide holistic care to this selected patient.&nbsp;<\/p>\n\n\n\n<p><strong>Reason for Inclusion of the Assignment in the Portfolio:<\/strong> I decided to include this assignment in my portfolio because of my ability to provide holistic care to a patient of a diverse population in a unique situation. Additionally, I wanted to show the importance of providing therapeutic communication to patients, even those who are nonverbal. I also talk about the importance of monitoring trends in lab values and vital signs for all patients.&nbsp;<\/p>\n\n\n\n<ul>\n<li><strong>Domain 2: Person-Centered Care:<\/strong>\n<ul>\n<li>End of program learning outcome 6: Communicate effectively with individuals. (Domain 2) (2.2, 2.3, 2.4)\n<ul>\n<li>In this case study, I discussed how I used nonverbal communication to communicate with this patient. The main form of communication with this patient is through therapeutic touch. Although this patient could not respond, I made sure I still talked through everything I was doing with this patient. I also communicated with the patient&#8217;s family members and explained everything I was doing with the patient to them as well.&nbsp;<\/li>\n<\/ul>\n<\/li>\n<\/ul>\n<\/li>\n\n\n\n<li><strong>Domains 2, 3: Person-Centered Care, Population Health Descriptor<\/strong>\n<ul>\n<li>End of program learning outcome 8: Applies appropriate knowledge of major health problems to guide nursing practice. (Domains 2, 3) (2.2,2.3, 2.4,3.4)\n<ul>\n<li>In this paper, I give information on five nursing diagnoses related to the patients&#8217; admitting diagnoses. I also used research and assessment findings to explain why I chose the diagnosis I did. For example, my priority diagnosis was ineffective cerebral tissue perfusion, and I chose this due to the patient being nonverbal, only responding to painful stimuli, and having an altered mental status. Interventions used to prevent further complications were administering Metoprolol to lower her blood pressure and open up her blood vessels, increasing cerebral tissue perfusion.&nbsp;&nbsp;&nbsp;&nbsp;<\/li>\n<\/ul>\n<\/li>\n\n\n\n<li>End of program learning outcome 9: Implements nursing care practices as appropriate to provide holistic health care to diverse populations across the lifespan. (Domains 2, 3) (2.2,2.3,2.4,3.4).\n<ul>\n<li>When providing care to this patient, it was important to look at the patient as a whole. This patient is an 86-year-old African American, making her part of a diverse population. She also lives alone, so she is at risk for social isolation. With this information, education with the patient&#8217;s family was important, and discussing the risks of this patient going back home alone. Communicating with the case manager was a big part of this patient&#8217;s holistic care during her stay.&nbsp;<\/li>\n<\/ul>\n<\/li>\n\n\n\n<li>End of program learning outcome 12: Establishes and\/or utilizes outcome measures to evaluate the effectiveness of care. (Domain 2,3) (2.2,2.3,2.4,3.4).\n<ul>\n<li>In this paper, I discuss two patient outcomes: one related to her nursing diagnosis of ineffective cerebral tissue perfusion and the other to the nursing diagnosis of fluid volume deficit. The first outcome I identified was that the patient would show an improved level of consciousness within 72 hours. The second outcome was that the patient will show signs of adequate fluid volume as evidenced by increased urine output, mucous membranes will remain moist, CBC levels will improve, increased level of consciousness, stable vital signs, and improved skin turgor. After these outcomes, we provided interventions and later reevaluated the outcomes.&nbsp;<\/li>\n<\/ul>\n<\/li>\n<\/ul>\n<\/li>\n\n\n\n<li><strong>Domains 2, 3, 5: Person-Centered Care, Population Health Descriptor, and Quality Safety&nbsp;<\/strong>\n<ul>\n<li>End of program learning outcome 11:&nbsp; Performs therapeutic interventions that incorporate principles of quality management and proper safety techniques. (Domains 2, 3, 5) (2.2,2.3, 2.4,3.4,5.1,5.2)\n<ul>\n<li>While caring for this patient, I provided therapeutic interventions to monitor their progress. Some interventions mentioned in this paper include neuro checks, monitoring GCS and vital signs q4hs, maintaining her fluid balance, and monitoring labs. All of these interventions will lead to greater outcomes for this patient.&nbsp;<\/li>\n<\/ul>\n<\/li>\n<\/ul>\n<\/li>\n\n\n\n<li><strong>Domain 4: Scholarship for the Nursing Discipline&nbsp;<\/strong>\n<ul>\n<li>End of program learning outcome 26: Applies research-based knowledge from the arts, humanities, and sciences to complement nursing practice. (Domain 4) (4.2)\n<ul>\n<li>In this paper, I used a nursing research article titled \u201cHow to recognize and treat metabolic encephalopathy in Neurology intensive care unit,\u201d to discuss how treatment of metabolic encephalopathy varies. Treatment varies based on the cause of the encephalopathy. My patient had encephalopathy due to a thiamine deficiency, so she was getting 100mg of Thiamine via IV.<\/li>\n<\/ul>\n<\/li>\n\n\n\n<li>End of program learning outcome 27: Evaluates research that focuses on the efficacy and effectiveness of nursing interventions. (Domain 4) (4.2)\n<ul>\n<li>In this paper, research related to metabolic encephalopathy was evaluated and then applied to patient care. My interventions, such as thiamine administration, fluid replacement, and neurological monitoring, were supported by current research and then tailored to my patient&#8217;s condition. The effectiveness of these interventions was monitored through patient outcomes, including laboratory trends, vital signs, and neurological status.   <\/li>\n<\/ul>\n<\/li>\n<\/ul>\n<\/li>\n\n\n\n<li><strong>Domain 8: Informatics and Healthcare Technologies.&nbsp;<\/strong>\n<ul>\n<li>End of program learning outcome 38: Uses informatics to enhance one\u2019s own knowledge base to provide patient-centered care and support teaching. (Domain 8) (8.1, 8.5)\n<ul>\n<li>I used informatics with this patient when using the EHR and monitoring her lab results, medications, imaging, and notes from other health care providers. Reading notes from other providers allowed me to see what each specialized department was gaining from the patient.
