Your Clinical Faculty will use the Lesson Observation Form for each lesson that you teach (8).  The Department of Human Movement Sciences Practicum Evaluation(369 CF final evaluation) form is completed by your Clinical Faculty at the end of your Practicum experience. The Practicum Evaluation by the University Faculty form is my evaluation of you and can be added to your ePortfolio at a later date.
