NURS 472: Community Health Nursing II

Purpose of the Assignment: Planning, Population-based Intervention and Evaluation. The purpose of this assignment is to utilize the Health Planning Model to improve aggregate health and to apply the nursing process to the larger aggregate within a systems framework.

Student Approach to Assignment: I approached this assignment by collaborating with my fellow classmates to create a plan of intervention for a population group identified. We worked alongside ForKids throughout the semesters and identified the need for oral health education among the population. We gathered research data to guide our decisions in implementing interventions. 

Reason for Inclusion of the Assignment in the Portfolio: The inclusion of this assignment in the portfolio is important because it demonstrates my ability to collaborate with organizations and identify needs across a population. As I begin my career in nursing, I will need to identify SDOH and factors impacting communities. It showcases my ability to demonstrate my practice outside of the traditional healthcare setting and applies clinical research to real application.

  • Domain 3:
    • (23) Prioritize patient-focused and/or community action plans that are safe, effective, and efficient in the context of available resources.
      • In this assignment, I demonstrated the ability to identify a community action plan that is efficient and within my available resources. A priority problem was identified, a community intervention was created, and having no long-term follow up was emphasized as an area in which we had to create an intervention based on available resources.
  • Domains 3 and 6:
    • (20) Initiates community partnerships to establish health promotion goals and implements strategies to meet those goals.
      • In this assignment, I demonstrated the ability to work with a community organization to establish a health promotion goal. The health promotion goal was oral hygiene education and these strategies were implemented by providing educational pamphlets, pre/post assessment quizzes, and providing teaching during mobile health clinics.
